J beach hMembership Fees and Form

The easiest and quickest was to renew your membership and pay is via Surf Lifesaving SA online. Log-in and follow the prompts to firstly renew your membership for the season and then make your payment. You can pay membership and any donations using this method. Remember to renew and PAY to ensure activation of your membership.  

It is a Surf Life Saving requirement that all members complete a new membership form each season.

Junior Come 'N Try Waiver Form

NOTE: FREE COME 'N TRY (not available on 8/11/2014)

At Henley we offer a free Come & Try session from the 18th October 2014.  Any after this time will be at the discretion of the membership manager - eg: moving from interstate or overseas.

Your child or children, are able to come along and participate in a Saturday "Junior" Session!!  You will need to sign a waiver form first before your child can participate.

This offer is available to children from the age of 5 to 13.  

We have a very strong Sun Smart policy at Henley SLSC, and your child will need the following - Broad brimmed hat (No Caps), Long sleeve rash shirt, bathers (NO BOARD SHORTS) and drink bottle.  Our policy is "No Hat, No Play".

Please arrive early (1:45pm) to fill out the necessary forms with our registration ladies.... and to meet our Age Group Leaders.

Junior Medical Condition Form

If your child has a medical condition that may affect their participation in Junior Activities (asthma, epilepsy, allergies etc.) please fill out a  medical form and bring it with you to registration.

Junior Photo/Video Consent Form

Henley SLSC at times may request to use video footage/photos/other images of our Junior members that have been taken by the official club photographer or taken by Surf Life Saving SA for a variety of public relations, communications and promotional activities, including for publications, promotional material, websites and advertisements, for an undefined period of time.

Before any photos are published the parental consent form is required to be completed.
